Policies to ensure safety, privacy and consent

Observation
If you are receiving a vaccine for the first time, Health Shots requires you to remain on site for 10 minutes after receiving your injection. This time is used for observation to ensure your safety.

Pregnancy
Pregnant women must notify their physician prior to any vaccination. Please notify your clinic nurse of your pregnancy prior to the vaccination.

Consent for Vaccinations
Consent forms and vaccination information sheets are supplied at the time of your scheduled clinic. Each participant will have ample time to read and question any information provided.

Children
Children must be six months of age or older to receive a flu vaccination from Health Shots. Children under the age of nine, must have also received a previous flu vaccination. A parent or guardian must sign a consent form for individuals under 18 years of age. Permission slips are not an acceptable form of consent.

Medicare
Health Shots is not a Medicare provider, therefore, no reimbursement should be filed or expected. A receipt is provided for private insurance.

Privacy
Health Shots is compliant with HIPPA regulations. Health Shots wants you to feel secure with your privacy and confidentiality while visiting our website or using our services. This website offers health information, but Health Shots does not answer questions about your specific medical condition or treatment and is not intended to be a substitute for medical advice or treatment. Continue to consult your physician. No information concerning your company, personal, or health will be released without written consent.


Documentation
Records of immunizations are recorded with the Colorado Immunization Informational Systems.  This allows access to your records from your physicians office, health departments, or any provider registered with the CIIS.
